Jack Lee Forman of Hendersonville, TN passed away peacefully on October 12, 2024 at Alive Hospice in Nashville.
Born January 31, 1935 in Cincinnati, Ohio, to the late William and Inez Miller Forman, Jack leaves a legacy of faith, kindness and music to his family.
He was predeceased by his parents and three brothers – Don, Ken and Bob Forman.
He is survived by his wife of 67 years, Sue (Huls), children Jeffrey (Karan) Forman and Susan (John) Bennet, one sister, Carol (Howard) Clayton, and sister-in-law Chris Huls. He was blessed to know and love 10 grandchildren: Mallory (Michael) Fiery, Stacey (Nick) Ater, Ben (Lauren) Forman, Bethany Bennet, Shawn (Amy) Andrews, Chad (Lisa) Andrews, Lindsay (Michael) Koehl, Ashley (Tony) Gray, Whitney (Bill) DuBois, and Tim (Rachael) Bennet. He had 20 great-grandchildren. Jack was also loved by MANY extended relatives and friends.
Jack’s deep faith was expressed joyfully through music. He began singing at church as a youth and eventually sang in the choir at Eastern Kentucky University where he met Sue. Jack directed choirs at Salem First Baptist Church (Indiana), Federated Church and Terrace Shores Church in Green Lake, Wisconsin, and First Baptist Church Goodlettsville (Tennessee). Although he was not a trained musician, Jack loved God, music and people, so he touched many lives through this ministry. Jack’s love of music extended to singing with the Miss Atlanta USO Show while in college. He also performed barbershop harmony with various quartets and the Louisville Thoroughbred Chorus. Now he is singing in the heavenly choir!
Jack served others and impacted many through his vocation as a high school teacher for 17 years and guidance counselor for 20 years. Upon retirement from education, Jack and Sue moved to Green Lake, Wisconsin, where Jack managed the greenhouse and landscaping crews at the American Baptist Assembly conference center. This was a natural “second career” for Jack, who always loved flowers and earned a Master Gardener designation. After ten years, Jack and Sue had worked hard enough and moved to White House, Tennessee, and eventually to Hendersonville.
Jack was a favorite of everyone who knew him, but he was a humble servant as reflected in his favorite Scripture: “Do not think of yourself more highly than you ought, but rather think of yourself with sober judgment, in accordance with the measure of faith God has given you.” (Roman’s 12:3)
A Remembrance Service will be held on Thursday, October 17th at 11:00 AM from the chapel of Sellars Funeral Home at Hendersonville with Pastor Lyle Larson and Tom Huls officiating. There will be a visitation with the family at the funeral home on Wednesday, October 16th from 4:00-7:00 PM and Thursday, October 17th from 10:00 AM until service time at 11:00 AM.
Interment will be at Crown Hill Cemetery in Salem, Indiana. A Remembrance Service will be held in Salem at the First Baptist Church at a later date.
